Registration for the 2011 Conference, Housing, and Childcare

• Conference Registration
  
  Conference Registration Includes:
  1. All sessions and session materials for adults and children
  2. Childcare for children under age 6
  3. Reception on Wednesday evening
  4. Breakfast, lunch, and beverage breaks on Thursday and Friday (Family and Friends Banquet tickets Thursday are extra)
  5. Friday Teen Dinner Dance, Tween Karaoke Social (no dinner), Adults with type 1 Dinner (with RSVP)
  6. Friday Dessert with the Faculty
  7. Saturday Family Day at the Coronado (BBQ lunch tickets are extra)
  8. Farewell Breakfast on Sunday
  9. One t-shirt per registered person
  10. Friday Off Site Day for teens (park ticket, lunch and snack included)

  Cancellations prior to June 1, 2011 will be refunded minus a US$50 processing fee. After June 1, 2011, no refunds are available.

  Online registration is now closed. You may register on site beginning Tuesday, July 5.

  Registration dates:
  

  • Early Registration is $310 per family through December 31, 2010
  • Regular Registration is $360 per family from January 1 through June 17, 2011
  • On Site Registration is $450 per family after June 15, 2010. Registrations will not be accepted between June 17, 2011 and the start of the conference -- you must register on site.
  • Individual Professional Registration or Single Person is $210 (CEUs are not offered)

• Housing Registration
  

  Disney's Coronado Springs Resort is located near Magic Kingdom® Park, EPCOT®, Disney's Hollywood Studios, and Disney's Animal Kingdom® Theme Park. Five swimming pools are on the Hotel grounds, including a huge, feature-pool complex with southwestern theme. A children's playground is on site, as is a large game arcade and fitness center. Watercraft and bike rentals are offered. A jogging trail winds around the 15-acre lake. Complimentary Disney bus transportation is available between the Hotel and all Disney Theme Parks.

  Newly renovated hotel rooms include 2 double beds or a king bed, refrigerator, high speed internet (fee for service), ceiling fan, coffee maker, wall safe, telephone with voice mail and data port. Wireless high speed internet is available in public areas of the hotel, including the lobby, conference center, and swimming pool.

  Room rates are $149 + tax. This rate is available from July 1, 2011 to July 13, 2011 for those families who might want to extend their vacation.

  Corporate Suites may be reserved for hospitality events or entertaining, subject to availability.

  For housing reservations, call Disney conference reservations directly at 407-939-1020. Be sure to say you are with the Children with Diabetes Friends for Life conference. The Children with Diabetes room block is available on a first-come, first-served basis. You may also reserve a room online at www.disneyurl.com/ChildrenwithDiabetes2011.

  Please note that you are only in the CWD room block – and only have the perks of being in the room block - if you make reservations through the link or through this phone number (i.e., not through the Disney 800 number or through a travel agent – they cannot access the CWD rooms).

  Also, please note that this year's conference backs up to the busy July 4 holiday weekend. The resort will be quite crowded and will likely sell out early. If you plan to vacation for a few days prior to the conference, please make your reservations as soon as possible.

  • Childcare Registration
    Childcare is limited to the first 100 children. You must fill out the registration form (Word or PDF format) and return it according to the instructions in the form. Childcare Registrations will be accepted only after you have registered for the conference.

    The Childcare Medical Information Sheet is required for every child in child care and due by June 10, 2011.
