Registration for the 2013 Conference, Housing, and Childcare

• Conference Registration
  
  Conference Registration Includes:
  1. All sessions and session materials for adults and children
  2. Childcare for children under age 6
  3. Breakfast, lunch, and beverage breaks on Thursday and Friday (Family and Friends Banquet tickets Thursday are extra)
  4. Friday Adults with type 1 Social
  5. Friday Dessert with the Faculty
  6. Saturday Teen Dinner Dance, Tween Karaoke Social (no dinner)
  7. Saturday Family Day at the resort
  8. Farewell Breakfast on Sunday
  9. One t-shirt per registered person
  10. Friday Off Site Day for teens (park ticket, lunch and snack included)

  Cancellation policy:
  Cancellations prior to June 1, 2013 will be refunded minus a US$75 processing fee. After June 1, 2013, no refunds are available.

  Registration dates:
  

  • Early Registration is $310 per family through December 31, 2012
  • Regular Registration is $360 per family from January 1 through June 15, 2013
  • On Site Registration is $450 per family after June 15, 2013. Registrations will not be accepted between June 16, 2013 and the start of the conference -- you must register on site.
  • Individual Professional Registration or Single Person is $210 (CEUs are not offered)

  The Diabetes Scholars Foundation offers scholarships for families and young adults to attend CWD's Friends for Life conference.

  Disney's Coronado Springs Resort is located near Magic Kingdom® Park, EPCOT®, Disney's Hollywood Studios, and Disney's Animal Kingdom® Theme Park. Five swimming pools are on the Hotel grounds, including a huge, feature-pool complex with southwestern theme. A children's playground is on site, as is a large game arcade and fitness center. Watercraft and bike rentals are offered. A jogging trail winds around the 15-acre lake. Complimentary Disney bus transportation is available between the Hotel and all Disney Theme Parks.

  Newly renovated hotel rooms include 2 double beds or a king bed, refrigerator, high speed internet (fee for service), ceiling fan, coffee maker, wall safe, telephone with voice mail and data port. Wireless high speed internet is available in public areas of the hotel, including the lobby, conference center, and swimming pool.

  Room rates are $153 + tax. This rate is available from July 6, 2013 to July 17, 2013 for those families who might want to extend their vacation.

  Corporate Suites may be reserved for hospitality events or entertaining, subject to availability.

  For housing reservations, call Disney conference reservations directly at 407-939-1020. Be sure to say you are with the Children with Diabetes Friends for Life conference. The Children with Diabetes room block is available on a first-come, first-served basis. You may also reserve a room, buy park tickets, make Disney Dining reservations, and arrange for free Disney Transporation from the Orlando Airport to the hotel online at www.disneyurl.com/Diabetes2013.

  Please note that you are only in the CWD room block – and only have the perks of being in the room block - if you make reservations through the link or through this phone number (i.e., not through the Disney 800 number, or through a travel agent – they cannot access the CWD rooms).

  • Childcare Registration
    Childcare is limited to the first 100 children. You must fill out the registration form (Word or PDF format) and return it according to the instructions in the form. Childcare Registrations will be accepted only after you have registered for the conference. The Childcare Registration form will be online in early 2013.

  • Childcare Medical Information Sheet
    The Childcare Medical Information Sheet is required for every child in child care and due by June 10, 2013.

• Conference Waiver
  Every family must provide a signed waiver that covers all family members attending the conference. You may print the waiver, complete it, sign it, and hand it in at registration, or you may fill it out at registration. The waiver will be online in early 2013.
