Registration for the Conference and Housing

• Conference Registration
  

  Registration includes:

  • All sessions and session materials for adults and children
  • Childcare for potty-trained children under age 6
  • Reception on Friday evening
  • Breakfast, lunch, evening reception, and beverage breaks on Saturday
  • Breakfast, lunch, and beverage breaks on Sunday
  • One t-shirt per registered person
  Online registration is closed. On-site registration will be accepted at the conference. Please phone Mike Schurig at 407-443-7966 between 9am and 5pm Eastern to confirm availability.
  
  

  Cancellation policy:
  Cancellations prior to November 1, 2011 will be refunded minus a $50 processing fee. After November 1, 2011, no refunds are available.

• Housing Registration
  

  The Renaissance Seattle is located at 515 Madison St. in Seattle, Washington on the corner of Madison Street and 6th Avenue. This is approximately 12 miles from the Sea/Tac International Airport. This property offers some wonderful space to CWD participants including plenty of meeting room accommodations for all age groups, an indoor swimming pool on the 28th floor, several restaurants, and a state of the art fitness center. Single and double room rates are $165 per night plus tax. Rollaways are extra. Each room offers high speed Internet, a 2-line phone with voice mail, cable TV, and a coffee maker with complimentary coffee. Within walking distance are the Waterfront, the Shopping District, and the famous Pike Place Market. Also nearby is the Monorail, which travels to the Space Needle and Seattle Center.

  To make your room reservation, you can register online or call 800-546-9184. If you call, you must identify yourself as being with the Children with Diabetes conference. Reservations are available on a first-come, first-served basis. The room block will be held for CWD until October 13, 2011. Note that there is a conference registration fee per family and that reserving a room does not register you for the conference. Rates vary according to the date of registration.
