Teen Adventure Camp and Triangle D Camp are held at YMCA Camp Duncan, a 400 acre setting on semi-private Fish Lake on Route 12 just north of Route 120 in Ingleside, Illinois (map). Parents are responsible for transportation to and from camp.Camp Education Programs
Teen Adventure Camp and Triangle D Camp combine a program of summer activities with a program of diabetes education and support. Both components are designed to challenge each child to reach toward their maximum potential, both mentally and physically. The campers are actively involved in managing their diabetes. Goals for campers include improved self-esteem, taking responsibility for diabetes management, and social adjustment in a new setting away from home.

The activities are planned for the interest and skill level of each age group and are designed to provide new and different experiences for campers each year.
Meal Plan
The camp meal plan is consistent with the current recommendations of the American Diabetes Association based on the exchange system and carbohydrate counting, with modifications in fat, fiber, and salt content.Cost
The cost for each one week session is $770 for ADA members in the Northern Illinois area and $870 for nonmembers in the Northern Illinois area.Please DO NOT let the cost stop you from sending us your child's applications--camperships, fee reductions and time payment schedules can be arranged.
Camper Registration
- The American Diabetes Association does not discriminate based on race, creed, national origin, gender, or special needs. Registrations/requests for applications are processed on a first come, first served basis** pending review/acceptance by the medical members of the camp committee.
- No child is denied the opportunity to attend the camp due to lack of funds.
- Directions to camp and a list of "what to bring" will be provided in the acceptance packet. For additional information, call Sue at 312-346-1805 or 1-888-342-2383, x-6567.
**First-come, first-served is defined as: the date the original forms mailed out are returned to the ADA office complete and ready to process. Facsimiles are not accepted.

To sign up your child online, please visit www.diabetes.org/camps. To sign up your child via paper application, contact the Northern Illinois Area for a Reservation form to begin the application process.American Diabetes Association

JWisz@diabetes.orgOnce your reservation form and deposit is received by the Northern Illinois Area office, the remaining application packet will be sent out to you. The application packet consists of several forms to fill out and return. The child's physician must fill out the physician's form and return in a signed and sealed envelope to the parent. If the child sees a counselor/therapist, a form is also required from their counselor and also returned in a signed and sealed envelope.
No facsimilies are accepted. Only complete applications will be processed and reviewed by the camp medical staff.
