It's the same as other CWD chat rooms - with a lot of people, though, it can sometimes seem disorganized and perhaps as if a dozen different threads are ongoing at the same time. Well, they are. It's Joe's job to keep talking about the serious content of our conversation. It's my job to keep track of the questions and topics and try to make sure nothing drops off. So that we are all on the same page, tonight's chat is about communicating with your family about something extraordinarily difficult - the death of a child with type 1 diabetes. Joe is aware of the several children who have recently passed away, and he is aware of our distress as a group and our collective/individual confusion about what to say and do.
Please note that this chat is NOT about 'what happened' to the children or the topic of DIB itself (I know it had been promoted somewhere as that, and I'm not sure where that came from.

Hi All. I'm April. Our son was diagnosed 5 weeks ago. He is struggling with fear every night before falling asleep. Afraid he won't wake up. He isn't recognizing his lows consistently yet. Having a hard time helping him with the anxiety. Joe we heard you talk at UCSF and it helped so much. Thank you for helping us with this tough subject.

Just wanted to say this about diabetes, kids and lows: It is very rare for a thing like this to happen. It is not the usual course of diabetes. Without saying or meaning to say anything disparaging about anyone, it usually happens in kids where things in diabetes management aren't going well

Well my daughter is older, so we had a different experience in telling her than most of you would, Plus she knew some of the kids. I was brutally honest. her reaction in one case (a dka) was :Wow, that could have been me. which is true, IMHO if younger kids don't know the kid and have not heard, why would you tell them?

Often my son will ask what will happen if the meter shows 0. I don't know what to say. I just tell him we'll make sure that never happens. He then asks will he be dead by then. I repeat my reassurance to him, but I wish I could say it otherwise.

it's important to make sure that they test before going to bed, if exercising, take an extra double snack, and check in the middle of the night. CGM with an alarm is something that can help alleviate the fear

As kids get older, they have the ability to recognize the symptoms. If kids have been really running very low for a time, they may have hypoglycemic unawareness, which can be corrected by running them a little high

Laura, the girls reaction was about the same. They know that I am here now and they just blow it off. But they ask me to test if they have been high or low. I test during the night anyway.

I told Jenny too, somebody was bringing up the point of bolusing for breakfast in bed, and then fall back a sleep. And because she like to prebolus in bed, I had to talk to her about it. She just took it as a fact of live. I am way more upset about it.

I think the older kids, as they hear the news, should have a discussion with you as parents. The message they need to hear is that that's why it's important to really be on your game and to help them come to terms with the idea that some things can happen EVEN if you are doing everything all right. We'll be there to hold them and help them without any judgement. And it's apporppriate t0o be scared about this.

I think it's best to ask the teen what their reaction was/is. Then ask them to expand. Then let them know that it's not that usual and then ask them what you can do to help reassure them, and then let them know that it's ok to be afraid.AND stay involved, ytotally in the things that they're doing

I don't remind Maddie of the risk of death by drowning every time she swims ... I just try to make sure that she has the tools to avoid drowning - ie swim skills, life guard, shallow water etc

I'm not comforted at all by the stats because ALL kids have the risk of fire/accident/etc. NOT all kids have Type 1. That puts a different perspective on it. Less kids die from Type 1 because not all kids have it.

Sam uses a cgm. It doesn't always work for him in the middle of the night (he occasionally gets 'noise' instead of readings) - so he still checks bgs at night.

What I need to learn how to do is to get away from saying a bg is good or bad or whatever. She KNOWS if it's good or bad and I don't want her to feel she has to start lying about her numbers so there isn't pressure - so much pressure already!

I think I am looking at it this way. while I am still doing night testing and looking after Sasha her levels and HbA1c will be ok but once she is on her own she will have to loosen control a bit for safety at night

I have mixed emotions about CGM. We wore them. But they don't make it any less necessary to do the behaviors. I think at time they become security blankets for us parents and just anothe hole in the side of the kids.

we have a CGM and it is a very valuable tool that does help us get more sleep. At least in teh sense that we can check it and go right back to sleep if all is ok instead of fully waking up to actually check a finger.

Diamond's Dex is a comfort. It didn't really lessen the finger sticks as much as I thought, but seeing the trends is a huge help!

We tried a CGM, but it didn't ever seem accurate. I couldn't ever trust it and found myself checking more often because the information it gave me was confusing. I bagged it and I'm skeptical of trying again anytime soon.

We have dexcoms. I think they work great for basal tuning. Dont use 24/7/365

We honestly love it and refuse to live with out it. Last night we had to do a new sensor so having the information was really annoying and made for a long night (we tend to have failures on the first night then after that it works great for up to 14 days)

Joe- Jesse and I have many discussions over the last week... He's going away to college - and I'm probably more nervous than he is... hi  Smile  jumping in late here..Eilish found out about the deaths on Facebook unfortunately, and is worrying about it, I can tell (i'd prefer she didn't know about this yet) So I reassured her that we test overnight to keep her safe, but i know the next logical question is going to be - what happens when she's older and living away from home. my probelm wiht my daughter is she not communicative lol we have tried counsleing several timesa she doesnt speak out loud in public and its a waste of time her best therapy is meeting her twinsie who lives on our street and her same age! and being a LIT at the barton day campo in boston even if we have to fly to mass for It LOL!

I think any thersapist will do, as long as they're creative, compassionate and willing to learn from you and ytour family what it's really like to have diabetes. That's what works for me and my therapist.she's awesome and has made an effort to understand what it's like for me.

I'm not saying therapy is for everyone or even neccessary unless you need it. If your kids are smiling, doin g their homework, enjoying friends, etc you DON'T have to worry that some unresolved issue is lurkiung underground

The best motivation my kids got was at FFL when Jim Vail talked to the teens about being average. Average person is half male half female. You don't want good average numbers.

The girls go to the camp and this will be their 6th year. At camp you either have to be 16 or have finished your sophomore year in school

I've just called Joe and asked him to please comment on how 'news' like this week affects different ages of children (say, a 5 year old vs an 8 year old vs a young teen vs an older teen) - how they might process that info at different ages. All I'm saying is to be open, open open - to what comes down the road, like this thing that we're talkjing about, and getting wise counsel from friends and family and managing it from a real perspective-honest, but not overly indulgent. That seems to be the hardest thing for all of us, to feel the fear, recognize where it's coming from and then say the Serenity Prayer! seriously!!!

How this kind of event and the news that follows can affect different aged kids differently-for 7-10 year olds, it can be frightening in a magical kind of way-their imaginations run and we need to help provide them with reality testing and grounding, to talk about what's scaring them but to come back to _you know how you feel, we almost all of the time trwt it in time and we're here for you. In addition to having great imaginations, they also, at this age really appreciate conctreteness. As they get older, it can affect them in a more obviously emotional way, and what's nice is the older they are the more you can have an adult-like conversation with them

the talk with the younger ones really needs to acknowledge their fantassies AND ground them in the concreteness that they so love. EG, it hasn't happened to us, we're always monitoring your numbers, that's why we work so hard at managing it, pointing out to them all the times that they have recognized sy,ptoms and successfully treated it AND congratulating them on their courage and bravery and then a good, sweet hug

Would like to comment about the immortality note - my guess is that the children who knew Trent or any of the others will have very different reactions than those who did not. I personally think it's important not to react to any number in a negative way. Kids don't want to worry us and if they think a high number will stress us out then they will hide it.

So - the 'facing mortality' - with social media, our kids are going to hear more stories about people dying (not just kids with type 1) - and it just adds to the sense of 'what the heck is going on and why does it seem like so many kids are dying?' I don't think it's more kids at all - I think it is our access to media. THAT scares me.

I'm joining late, but it seems that our kids identify with their diabetes TOO much....like it is their identity and they are cheering it on. Has anyone else noticed this? It seems important that our kids understand they they ARE NOT their disease...that they need to be in charge of IT and not the other way around.

I agree gboysmom...I understand that we're working towards communication, but all we know is that kids are dead. WHAT HAPPENED? And I realize that's not what we're here to discuss, but how the heck am I suppose to talk about it with my child if I have no idea what happened? What kills children with diabetes? What does the research say? Is it a pump problem? Are there batches of faulty insulin, strips, METERS THAT ONLY NEED TO BE WITHIN 20% accuracy? There's room for all out panic and I'm trying to avoid that. So what does the research say? Why do otherwise healthy CWD die???????

This was something Joe said - think the older kids, as they hear the news, should have a discussion with you as parents. The message they need to hear is that that's why it's important to really be on your game and to help them come to terms with the idea that some things can happen EVEN if you are doing everything alright. We'll be there to hold them and help them without any judgement. And it's appropriate to be scared about this.

Yes, I guess so. Joe was saying not to try to scare them, as scare tactics don't work. I understand that, but I think it's hard when you are asked why they died to say one was from a low and the other a high... that is just scary.

Very disappointed The Endocrine Society only asked FDA for blood glucose meters to be within +/- 15% margin of error instead of 10%

I don't know how it would be discussed, Ellen. This was 70 people in a chat room. Can you imagine 1000 parents in a ballroom? I don't think much would be accomplished. Send me a note about the Red Flag idea. Our schedule is 100% entirely full for FFL this year, but I am already planning next. btw, Arleen Pinkos from the FDA will be at FFL again, talking about meter accuracy.

Ok there is a reason behind this question, because we are having a bit of a problem with it currently. What do you consider a number low enough to cause some concern for a waking number in the morning? Hope that makes sense.

I think it is different in each case----at the point there is an 'unawareness'.......Kaitlyn used to wake up at 80 all teh time...ai 60 she can wake up and go and get something to eat....some people would be horrified at that number....it is not thenorm but it happens and she eats something and moves on.

Know that given FDA standards - a 50 can acceptably really be a 35 (+/- 15 mg/dl if bg is lower than 75 mg/dl - and that's 95% of the time. The other 5% are outliers)

doesn't seem to matter what the bedtime bg is, need to look at the cgms graphs to see if there is a pattern during a certain time, been working all week and haven't had a chance to do that. By the time I've been getting in Paige is in bed.

That's why I think 80 is a safer number. You may want to raise the number at night for the alarm - although I know it can be so annoying going off all night long too

Most people know their BGM....but I would not want to advise you---very dangerous practice---it is something you should work on with your family and your endo.....what our numbers are in relation to everything may be completely different with what you ar edealing with.

that's a double sided coin, "actual" waking number? between 70-80, but how do we know what is actual? What would be considered dangerous? What would be low enough that she would not wake up? Too many ifs in our world

DD, we do talk about it with our endo, but that's what we are here for as well is to give others ideas. Some advice sometimes is needed for people, that's why they come here. However, with that being said, I didn't as for advice on what exactly to do, I need to find the problem and I'll know what to do. The questions that I had are what other people feel to be comfortable numbers or uncomfortable with, simply an opinion.

Tomix, that's what prompted the question, she has went to bed anywhere from 100 to 220 and still waking with lows. With our cgms, us and our diabetes team are fine with her going to bed between 100-120 but we need to figure out what is going on in the middle of the night
