Hi Natalie, Marissa is playing soccer. Just had practice this evening, for 90 min. She never disconnects, but must run a separate basal program that begins at 10p.m., with reduced basals until 3 or 4 a.m.

Hello, When my daughter age 5 plays, esp sports we have problems with her going higher if she is at the top of her range, 200. why we expected her to go lower, any suggestions?

Marissa is also running a new "regular" basal program. She has aerobics at school in the mornings and was having lots of lows around 5 p.m. We seemed to have worked that out, but it came around the same time "all hell broke loose"---for some reason, she needed much less insulin, ratios upped (or lowered??) to 1:20 instead of 1:15 and much less insulin for corrections. .5 was dropping her almost 100 mg/dl instead of 35 mg/dl. We had a very stressful few weeks.

Wha is she doing for exercise, how long is she playing? and what are you doing wih her pump?

She just doubled her exercise! All of the ratios changed for a reason... Her body needs much less insulin because she is a lean mean exercising machine...

This usually happens when playing soccer/running alot, 20 minutes, we currently leave the pump on with no adjustments if she is about 200. If 140 or lower we decrease the basal or take the pump off(if she'll let us)

Becky, sometimes when Marissa starts a game with a higher BG, say 150 instead of 100, her BG will also be higher at the end. I think it's adrenaline causing the high. Is this possible, Natalie?

we've been having problems with chris's bg's skyrocketing abpout 2 hrs after practice...hr runs cross country and disconencts, the other day he was 150 at start, 127 at end, and 2 hrs later (with no foodd) was 426

Hi, we have been on the pump for about 1 year now, Jae is starting to play basketball, should she disconnect or is it okay to play with the pump on??

Kathleen if your son is off the pump for 2 hours his high BG is from NO insulin on board...1 hour is all we like to push it..the firs result is always OK it is the following one that we see and know the pump was off too long...

but if we leave it on, we get bgs of 40's after exercise...we've tried decreased basal and for some reason it just doesnt work...what percemt to you recomend? weve done 40 and it hasnt worked- maybe we just havent tried it frequently enough

she tends to be high after 2 hours of practice, we usually check her BS before and after practice. Is this normal?

the complicated part is its running...so it can be challanging to eat a snakc before or check while in the middle of a race, as all is timed

How high is she going? Typically people go hypo after...are you cuting back too much or is she having undetected hypos?

As we all know highs are safer than hypos. We like to sart treating bby geting consistently slightly hyper BGs hen reat downwasrds. Patterns are difficuly, but no impossible wih exercise. Remember to be patient and that one day does no a pattern make.

200 is Rachaels upper range so so far we have not been correcting unless she is 220 and up. This is our first experience with d and sports

I have a question...why is it that I see all of these other "younger kids" on the pump, but I have been fighting with Endo for a year now to have my son put on?

Becky remember insulin is the hormone that tells the liver to stop making too much sugar. You can 'slightly' or 'gently' treat a 200...

Some people need a slight INCREASE in basal after exercise---as little as 10% for 2-3 hours will make a difference...but you have to check with you MD...

Marie Chris is Jesi's age - pumping has been great for us but its not for everyone, we almost felt like we were being pushed to pump before we were ready, but so far so good

Kahleen--keep in mind that we need him to be doing all of this by college. Give him litle bits of responsibility---but be willing o absorb it back as he is just a child. He needs some experience and to be able to sleep at friends homes,but the CWD saying os let your kids be kids...they all understand the saying you will do his by the toime you get to college...

How much should I bolus for Ty before soccer practice,{hard running] when I give him his snack. I think I remember Rick saying he needs some insulin, anot to just give him his snack with no insullin, but he keeps going low at half hour. I've been giving him a snack with half the bolus.

Marissa was in 6th grade when she got her pump; she's now a high school jr. and does most of her own care. I still do nighttime testing, unless she's at a sleepover; I will call her to remind her to test.

Brenda, she can add juice, bu we never want to start by feeding the insulin.... So, if she just wants him to eat the snack we give him 1/4...if he wans or is allowed juice we give him 1/4 for all the total carbs...So things are consisten...

Marie-we need to talk about Jesi live as I need to do a full assessmen on her... Someimes pumps help burnou...sometimes the initial work can be overwhelming even if i only for a very short period of time

One always needs insulin on board...sometimes more during game if you are dealing with an adrenaline rush, someimes less for aerobic no adrenaline workout

Well, I think wearing it is up to you/him. Marissa would need to reconnect or get a shot at half-time because her games are 80 min. long, not including the 40 min. warm-up and half. That's way too much time to be disconnected.

Christina, I believe Marissa would develop ketones. I think Ty's games are much shorter and he should not have the same problem.

Megan's friend (also Type 1) had her pump off 1 hour for
gymnastics. Her b.g. was fine, but had ketones already. The Endo had asked them to check when they inquired about how long was okay to be off the pump.

I think, though, that some people are more prone to ketones than others; Marissa doesn't get them very often, but I've heard of others who get them all the time, even if their BG isn't that high.

Natalie, thanks for the tip on using Dial soap after Emla cream (from the Florida conference). It's so nice not to have to use tegaderm on top. Even for swimming!

General rule of humb is one hour to aviod ketone...I am surprised they were present already... It could be they were present before she started or anaerobic exercise...

Ketones are just a sign that your body has used up the insulin on board. Absorption is different for all people...so ketones present from 1-3 hours after removing pump...one hour is pretty rare, though...

But, don't ketones associated with lows "usually" mean it's ketosis, not ketoacidosis, often from insufficient carbs like during an illness?

Brenda, no matter how hard Jesi and I try there is just no getting around the extreem highs and lows....it seems as if he always has "ketones" present:(

If he is not hypo befpre, test him at the half and treat to 150ish...either by feeding 5-15 grams carb or giving ~1/2 they dose to treat the hyper...

Emal/Elamax advice---cleanemla or Elamax with dial soap befor inserting catheter. If you do not remove ALL OF HE EMLA OR ELAMAX your catheter wiill mot say in for more than a day...

Marie, Marissa goes to Children's in Cincinnati. Her endo. is ok, but we switched to him after she was pumping. I have heard of other local families who have gotten pumps thru Children's in Cincinnati. I don't know if it's worth pursuing until after you talk more with Natalie.

we live in CT and go to Yale...we have a problem wit one nurse who we dont see, however she is the primary person who runs the clinic. these are prents of kids in town, and many have tried to leave and end up back at yale...Im not sure whether its worth it to try or if we should go with the majority of experiences
