my father in law was just diagnosed boarderline , bg's 150, they want to start with diet and exercise, I only have experience in feline diabetes, where do we start? no direction was given to him, he feels he has just been handed his death sentence, his father was diabetic, and his brother is also. any advice and guidance would be grateful to us!!

Francine ... our focus is on families who have kids with diabetes, and that's my personal experience. However, I would advise your father in law to take the exercise seriously. Exercise lowers blood sugars.

Debbie, this is not really a "diagnosis" question, as myself and my kids have had D for a fair while now! However, my youngest recently started school. She pumps, and prior to school was no problem with it. Now though, she is very self conscious, and does not want it to be seen. Any suggestions?

Sam's mom. That is a hard question but I think he should be allowed to do everything that other kids with diabetes can do. Can you train an adult going on the trip to help him?

I have tried to get a hold of the teacher for 3 days. They leave tomorrow. I guess I wouldn't mind if his brother was going too. He has always been the one to take care of Sam if something goes wrong.

Her close friends-the one who come for playdates know. Mostly they know because as Amber is being tested or treated for a low I field the questions that they have but Amber is not happy when I do.

Cass I guess it depends how you lost the weight. The healthiest way to lose weight is with a combination of a healthy diet and moderate exercise. Maybe your doctor thinks you are overdoing it with exercise?

Melanie-at the beginning of the school year Caleb's teacher talked to the class about diabetes, so they'de understand ehy he needed to leave the classroom and have extra snacks and not have cedrtain snacks, etc. But she also talked about the little boy with asthma, and another little boy who has a fake leg. So she didn't completly single him out. Then the kids all talked and asked questions.

Melanie- I have heard that many parents go into the classroom and give a little "presentation" about diabetes- read a stroy about diabetes and then take the time to answer any of their questions...kids are so curious at that age anyway

Cassy that is great advice about going into the class room. You just want to make sure that the child is ok with that as well. I would suggest you practice/role play before you go into the class.

Yes- we are fortunate that Nate has met many children with diabetes and as a result, he is not self-conscious about it (yet!)

Jeff to answer your question I do think kids experience the diagnosis of diabetes differently than parents. I think kids/teens worry more about the day to day changes and parents worry much more about the long term.

They did touch on at the beginning of the year so the class knows about it... and no one has teased her about it. She is just not comfortable answering questions that people:ie does it hurt.

I think teaching other kids in the class is a great idea if it is down age appropriately and if the child is ok with it. Sometimes reading a story helps.

Getting them in contact with someone in there area...we had a phone call a couple of months ago asking if our doctor could give out our phone number to any newly diagnosed type 1's in his care.

Debbie ... when Marissa was diagnosed (1989), there was little in the way of family support. We learned the mechanics of diabetes care, but not much about living with diabetes.

Our diabetes nurses asked us if we felt it was okay that they give our name to any newly diagnosed families (with children around the same age as Nate) in the area where we live- we have met a few very nice families and its been nice to help with there questions- its so overwhelming when you're just starting out

We were supposed to have an expierenced parent and child come to our house after diagnosis, but it never happened

Jeff I think family support is so helpful. That is why I think support groups are such a great idea. JDRF also has a program that matches newly diagnosed parents with a parent of a child with diabetes in their area.

We have some laws here that prevent doctors from giving out the names of others, but there must be some way to get the newly diagnosed in touch with more experienced families. Maybe endos can invite groups of families to discussion nights??

But we did have several visits from a home nurse over the first couple months. And we have a nurse who calls every few weeks to check up on him, and we can call her 24/7

I think there is still too little emphasis on the emotional aspects of living with diabetes when compared with the physical care. I believe every office should have a team approach that allows for calls when things feel out of control emotionally, not just the blood sugars. Why do you think in 2005 it's still often overlooked?

Brenda you are right. We can not give out the names of families because of HIPAA. But at support groups I always encourage families to exchange contact information.

After Sam was dx'd we had no calls or home visits from a nurse, but I did have to call his endo daily for the first month. He connected us with PADRES. It has been a great support group for Sam.

Ellen I agree with you 100%!!!! I am lucky where I work that we have social work support (myself) and we also have a pediatric psychologist. But I know that not all diabetes health care teams have family support staff.

Jen that is too bad that Children's doesn't have support groups. You should encourage them to start some. At the Joslin we are lucky to have groups for pre-school age kids and parents, school age kids and parents and for teens and their parents.

I'd like to see a conference exclusively on the emotional well being of the patient with diabetes. I think it's impossible to get through the physical aspects without the positive self-talk it requires to live with this 24/7/365

We put an ad in the free community happenings section of a newspaper "support group for parents of small children with diabetes and their families" and got phone calls and eventually started meeting at parks, homes etc. Eventually one person got us a room at a local hospital and then they started providing a facilitator etc. There was nothing in place before that.

Debbie, I want it for the patients and families, but certainly the health care providers need it too.

We try to limit our groups to just Joslin patients so that we dont contradict what is being said with your current provider. If you daughter loves her doctor there that is what is important.

CWD conference saved my relationship with Zack - Dr. Rubin said "when your child tells you his/her blood sugar is high, there is one thing and only one thing you can say in return 'THANKS FOR CHECKING' " Then you work together to troubleshoot to get the blood sugar down. That has been the best piece of advice given to me in 16 years of Zack living with diabetes.

Kate I have to agree with what you said. And I think you would all agree that this is not easy. But you need to make sure that your kids have healthy ways to take out their anger. I meet with so many kids and teens that dont know what to do with their emotions.

jeff I definitely work with the whole family. there are so many sibling issues that come up - fear of getting diabetes, anger that the child with diabetes gets so much attention, etc

That sounds familiar....we always try to focus on the fact that even though it does suck to be diabetic, there are so many other things that could be wrong-like legs that don't work, arms that don't work, or a broken brain, and that even though we would all rather she didn't have D, at least Amber can still do whatever she wants,and become whatever she wants. She is so kind and considerate of those who are limited in regards to what they can. She has only found one thing she cannot do in life-be a contestant on Survivor LOL

NAte is in an inclusion pre-school program at a school for kids with special needs-mostly metally retarded, cerebral palsy etc. Nate is considered a "mentor" to these children- he loves it

Amber would like to say that D is sometimes fun because if I am low I can have candy LOL She doesn't like D because the injections sometimes hurt, and the testing sometimes interupts something fun..like recess.

we are going to a one day event in march so she can see the camp. she is still insisiting that she wont go to sleepaway camp. well see after we see the camp. its easy to say just send her off to camp but she doesnt communicate well esp with people she doesnt know

I think the camps are so great for parents as well. First of all it gives you a much deserved break. But also you get to see other healthy children and teens with diabetes. A lot of the counselors have diabetes and they are healthy and athletic, etc which is very powerful for the parents and kids to see.

The important thing is to get help if you are feeling depressed. Depression is very treatable with counseling and at times medication.

I can speak to that. I have had 2 children with diabetes. When my second one was dx'd I could not breath and had to go to a couselor.

Things are totally out of control. Doctors often make it seem like they expect the unattainable - tight control , control the numbers....even when you meet other physicians and they learn your child has diabetes, they always ask "do you have it under control?" It sometimes takes years to get the courage to say 'we do the best we can each day'.

Jeff when I meet with a newly diagnosed family I try to keep things as simple as possible. So the main things I stress is support - like finding someone else that will learn to manage diabetes (other parent, grandparent, etc). I also stress self care. Because parents need to take care of themselves if they want to take care of their kids (much easier said than done).

Having help would relieve alot of stress. I'm the only one who takes care of Caleb, unless I have to work, then my parents handle things. But I never ever just get a break.

Ellen I hear your frustrations. Blood sugar control is almost an oxymoron. The key thing is aiming for a range. It is impossible to get perfect blood sugar control all the time with the tools we have. It is much more realistic to aim for a range most of the time.

Well I've learned a lot over the last 16 years, including finding lots of humor with the diabetes. If we can't laugh, we can't have a good time. It doesn't mean we aren't serious about taking good care of our child, but we all have to find something funny during the day.

That's not too bad. We had Nate one anywhere from 12-18 shots per day depending on. Almost hourly. The pump has been wonderful and freeing up alot of time.

She likes that part! Marissa was here for 3 min. She babysits every Thurs. for a family about 1.5 miles from here. Their 6 yr. old is a pumper!

Jen, They are not for everyone. I don't mean to have you think they are. Nate is 11 and a bull in a china shop so I didn't think it would work either. We went against Dr. better judgement and it's working for us. We are lucky.

Marissa drives to most of her babysitting jobs, which I find "different," but must be some modern thing (showing my age!). She sits for 4 families who have younger kids with diabetes.

question for all... my son has diabetes now for about 3 years now. and the last two days he has only had 1 unit of humalog. and he has had lows. he takes 5 units of lantos. isnt it odd for his sugar to be staying so good. when he use to take at least 5 units a aday humalog.....
