Joyce, that's very cool. It's hard to say exactly what will happen because different programs/educators go about the pump start process very differently.

There are a lot of tasks and teaching that need to be in place -- it depends to some extent on what Justin alredy knows (carb counting? Humalog adjustments for meals?)then th other things (how to operate the pump, insert the infsuion line and so on) are done in different orders by different educators.

Joyce, I would suggest you call back and ask those very questions so you and Justin know what to expect and are not disappointed if this is just a "get to know the pump" visit rather than start the pump visit.

He likes it because he says he feels "normal" again. His numbers have been really good. He has a lot less lows and highs and D was easier to control when he was sick.

One thing I am always trying to deal with is my fear of the future. Cam has graves disease also (a year after his D diagnosis) and I wonder why some people go for years without ever having a problem and some have many problems.

Ann, the pump is a personal choice. It's something you need to speak to you endo about to see if it is something that you would feel is right for you.

Karen, people with type 1 diabetes have more risk for other endocrine problems (like the Grave's) but this is not any sort of a marker for furture complications -- just wanted to make sure you knew that

When you are having such a struggle with bouncing blood sugars, I'd suggest talking to your team about giving the Humalog AFTER the child has eaten, and basing the dose on what was actually consumed. This generally works much more smoothly than trying to make a three year old eat a given amount of food to cover insulin that's already been injected.

cw, why do you use regular? I've heard that you can get better control with it. When you say you have pretty good numbers, what % of the time would you say your 3yo is in the range of 100-200?

I didn't know that at first Betty. He was hospitalized (DKA), Graves (with the eye bulges)within the first year. It scared me to death, but I just hoped it wasn't a prelude.

Kim, have you had the chance to check Ryan's blood sugar after meals? Maybe the background insulin is too high causing the lows or maybe the mealtime humalog was too much... lots of possibilities

cw, it's true tht in little ones who still eat substantial snacks between meals, Regular can work -- but I'd suggest that you get your hands on Sweet Kids and learn how you can move food around between the meal and the snack -- this can help trakae some of the pressure off of try to stick with the same meal plan everyday -- when kids appetites vary so much

cw - yes, pretty much. If anything, I feel like it is one variable in this whole puzzle that I can keep at a constant. Ian is a good eater, so that helps tremendously.

cindy - agreed, carb counting. sort of - we never really got a plan. the endo is very into 'adjusting the insulin to her' as opposed to 'feeding the insulin'. but that wont work when she sometimes eats a ton of cucumbers for lunch and sometimes noodles. so ive sort of been carb counting on my own.
betty - i read sweet kids :) and definitely do move food around between meals and snacks. we do not like to push food.

cw -- what a gift to have an endo with that outlook -- for little ones, sometimes a "sort of carb counting" approach that may help is to rate meals as "small, medium, or large" in terms of their carb count -- it's training wheels for getting into counting servings or grams.

marcello's mom -- a vitamin pill is a pretty cheap insurance policy -- especially if little ones are picky eaters or haven't developed their taste yet for a wide variety of foods -- look for one that is a "complete" supplement, preferably one that includes antioxidants, since these are probably of some extra value in diabetes.

funny thing betty - when my brother babysat i gave him a list of foods - with high, medium, or low, marked next to them. we used to do that, now i'm paying a bit more attention to portion size and nutritional info

The type 1 prevention trial is a great project to support and it will also answer the question of whether other family members are at risk of developing type 1 diabetes. Being at risk does not mean that they will definately get the disease but it is exciting research for all.

Philomena, there is an unusual concentration on this site of people who have multiple children with type 1 diabetes -- that's actually fairly rare if you look at ALL kids with type 1 -- so the odds are quite small that your other child will also develop type 1 -- doesn't mean impossible, but a small likelihood.

Any suggestions on how to get Matt to change from the stomach site to hip? He does not have much fat anywhere to insert so the small area on his stomach has been used up but I think he is afraid to use his hip.

Marcello's mom - there are several tests that can be done -- allare blood tests -- some look at anti-islet cell antibodies, other at some other proteins and some to determine the "haplotype" of the child -- don't ask me what that is!! just a way of charactering genetic make-up that has known relation to risk of type 1

The type 1 prevention trial has been going on for over 10 years now. My family was part of it years ago since my sister in law has type 1 diabetes. It is focused on finding the small numbers of people at risk of type 1 and then putting them into trials of low dose insulin, an insulin "pill" and no therapy to see if and when they will develop diabetes. Exciting stuff to be sure!

Cindy, what infusion set are you using? if he's lean, something short is called for -- like the shorter version of the Sof-set. That might make the process less dauting for Matt -- have you asked hin why he doesn't want to use the hip?

Ryan is on regular insulin shots in arms and legs . I am unsure about the pump for him he is petrified of sticking something important in tummy. He is really skinny.

kim, the 150 is mg/dl and that's the number system we use in the US. Most of the rest of the world uses mmol measurements which have decimals. To turn our numbers into mmol, divide the number by 18. 150mg/dl = approx. 8.3mmol.

Yes the girls help each other, but it was really hard on jessica when we found out cause she knew what she was in for. Marissa was dx'd at the age of 24 months and we talked very briefly about whether or not to have other children, knowing there was a slightly higher risk that they could also have diabetes. Clearly, we decided to have 2 more children and neither has D.

Florine was just diagnosed. Her father has type 1. She is one of three grandchildren and the only one diagnosed so far. There is a family history of type 1 on her father's side and type 1 and 2 on her nother's. We are devastated.

By the way, since I was reading back to catch up, I have to say to whoever was discussing pump sites in hips--it can be done. Marissa used her abdomen only for almost 2.5 yrs. About a month ago I urged her to try her hips. She won't rotate back to her abdomen at all--says the hip is perfectly comfortable. In sum, she was very pleasantly surprised.

The DPT will tell you if your child might get D some day. However, they cannot say when and they won't tell you if they give your child a placebo or not. There's a family near us whose oldest and youngest both have D. The middle child tested positive several yrs ago and had been taking low doses of insulin. I don't know her current status.

CW, it is possible to have type 1 diabetes and be set up genetically to also get some of the type 2 syndrome such as cholesterol problems, blood pressure problems and weight gain. They can benefit from some of the new medicines to treat those things and keep taking the insulin as before.

Thanks Betty. I will be very involved with her care. I do clinical research myself in cardiovascular disease and know all the complications and am therefore well aware of the consequences of poor control. The family is being tested genetically and hopefully this will help.

Karen, since my kids have been neg, I don't know the specifics. I know of one mom whose daughter has gotten some type of shot and takes various supplements/vitamins to prevent full blown onset. Her oldest has been taking the vitamins, too, and tested neg. recently after an earlier positive. Her middle child, a boy, has had diabetes for several yrs.

We LOVE the Minimed. before pumping we thought waterproof would be important,. Lauren is on a swim team. But you know what? It's a pain having ap ump hooked to you while swimming -- disconnecting would be our choice even if it was waterproof

We plan on doing a bribe soon to see if we can get her to use her legs, but need to stick with backside for awhile as we're doing a trial--- she is pumping Humalog with methadexazone added to see if the steroid addition will prevent lipoatrophy, a problem which is very worrisome

Beth, although the Dis. pump is waterproof, Marissa disconnects to swim. I tell her to test her BG, snack and bolus (reconnect or use her insulin pen) every 90 min. when she's at the pool. It's kind of hard for her to remember to do everything when I'm not there. Since Ian is much younger, you'd always be there to ensure nothing in forgotten.

Marissa started using her pump in Oct. '99 and it alarmed about 2 months ago. According to the manual, it need to go back for a "Technical Inspection." With Dis. you have a back-up and that's what she's been using for about 5 weeks. Luckily, we never needed it before. I have heard that some people have problems from the get go and have to ship their pump back after day 1. Of course, the same thing can happen with a clock or coffee maker.
