Welcome to the Type 1 Virtual Support Group. I'll be moderating on my own tonight. Kris is off running around. All topics and questions are welcome! We'll be starting in about 5 minutes.

Hi Betty :-) can you give me tech support on my cable connection?

Good night everyone!! Hi Betty!!!

Wed Jun 20 2001 20:59:04 Eastern US
Daniel (Pumper) from 64.20.226.143
any cures around the corner? and if so how long?

Wed Jun 20 2001 20:59:51 Eastern US
BettyB (Support Chat Moderator) from 198.81.16.49
Looks like my responses fell into cyberspace! Sorry, Daniel, I''m definitely not the techno whiz at the Center. Hasn't the pump company been helpful? Hola, Nina! Como estas?

Wed Jun 20 2001 21:00:53 Eastern US
Nina (Diabetes Educator) from 148.221.179.58
Hola Betty!! Muy bien gracias a Dios y tu?? Iīm in Cancun now! Will you come and visit me?? Mi casa es tu casa!!

Wed Jun 20 2001 21:00:56 Eastern US
BettyB (Support Chat Moderator) from 198.81.16.49
Daniel, I think we're going to have better treatments before we have a cure -- but it continues to come in slower steps than anyone would prefer. The artificial pancreas will undoubtedly be a reality while we're all still breathing!

Wed Jun 20 2001 21:01:39 Eastern US
BettyB (Support Chat Moderator) from 198.81.16.49
Cancun?? Que lastima!! You poor thing -- I surelt wish I COULD come visit you. Have you moved there now?

Wed Jun 20 2001 21:02:17 Eastern US

What kind of pump are you using Daniel?

Great!!! You will be welcome!

Wed Jun 20 2001 21:05:08 Eastern US
BettyB (Support Chat Moderator) from 198.81.16.49
The limiting factor on the closed loop system - as you probably know - is an internal sensor that will keep working in the body for a reasonable length of time -- I think they're up to about 2 days now - we're getting close but it won't be next week - or probably even next year.

Wed Jun 20 2001 21:05:08 Eastern US
Spensers mom from 24.218.5.194
We have been type 1 dxd from 11-21-00, when can we start seeing in range numbers?

Wed Jun 20 2001 21:05:50 Eastern US
Beth (IL) (Ian, 3 1/2, dx'd at 2 1/2- starts pumping next month) from 65.6.41.52
How old is Spenser?

Wed Jun 20 2001 21:06:23 Eastern US
Daniel (Pumper) from 64.20.226.143
what about a sensor such as minimed has? can't that be used in a closed loop system?

Wed Jun 20 2001 21:06:29 Eastern US
Spensers mom from 24.218.5.194
Spenser in now 9 years old, Five feet 2 1/2 inches, and 136 lbs

Wed Jun 20 2001 21:07:29 Eastern US
BettyB (Support Chat Moderator) from 198.81.16.49
The pump with Humalog is th best we have now, for type 1 -- the big shortcoming is that it delivers insulin into the subcutaneous fat, not into the portal circulation.. the closed loop system will solve that -- but I also understand there is work being done on placement of a cannula through which externally delivered insulin could be administered -- getting close to portal delivery.

Wed Jun 20 2001 21:08:05 Eastern US
Beth (IL) (Ian, 3 1/2, dx'd at 2 1/2- starts pumping next month) from 65.6.41.52
I can't speak for that age group - Ian has had D for 13 months, and his numbers are generally better than last fall, but still all over the place!

Wed Jun 20 2001 21:08:24 Eastern US
Daniel (Pumper) from 64.20.226.143
big nine year old :-) you must be taking good care of him! :-)

Spensers mom from 24.218.5.194
I wanted to know about the pen, we do NPH and R in the same needle, do I need 2 pens, or can I mix?

Wed Jun 20 2001 21:08:57 Eastern US
Heidyn (pumper 2 months) from 164.106.22.64
Hi

Wed Jun 20 2001 21:09:36 Eastern US
Beth (IL) (Ian, 3 1/2, dx'd at 2 1/2- starts pumping next month) from 65.6.41.52
We used pens for a little while - you have to do separate shots.

Wed Jun 20 2001 21:10:57 Eastern US
BettyB (Support Chat Moderator) from 198.81.16.49
Spensers mom -- you should have had time to achieve close to normal numbers by now. Do you know if that's what your doctor is actually aiming for. Some docs are less aggressive about control in youngsters than they would be in teens and adults. What treatment is Spenser on now?

Wed Jun 20 2001 21:11:05 Eastern US
BettyB (Support Chat Moderator) from 198.81.16.49
Hi, Heidyn!! Spenser'sMom, there is no mixing pen at this time. One was researched by one of the insulin companies, but never brought to market. You'd need to use a separate pen for each type of insulin.

Wed Jun 20 2001 21:11:05 Eastern US
Spensers mom from 24.218.5.194
Thanks for the pen info.

Wed Jun 20 2001 21:12:13 Eastern US
Nina (Diabetes Educator) from 148.221.179.58
But what about mix 25 Betty? how do you think it works?

Wed Jun 20 2001 21:12:51 Eastern US
Beth (IL) (Ian, 3 1/2, dx'd at 2 1/2- starts pumping next month) from 65.6.41.52
Heidyn - any words of wisdom from a new pumper? We are excited to start Ian, but nervous, too.

Wed Jun 20 2001 21:13:05 Eastern US
BettyB (Support Chat Moderator) from 198.81.16.49
There are so many things that effect blood sugar, perfect numbers are not possible with today's insulins and delivery systems -- but close to normal most of the time is absolutely possible. Are you trying to get Spenser to eat to match the insulin or is the insulin being adjusted according to what he eats?

Wed Jun 20 2001 21:13:19 Eastern US
Spensers mom from 24.218.5.194
Spenser has 29NPH and 7R in the am, 14NPH and 5 1/2 R PM, we do hit good numbers some times in the day, but also still hit 300's, I want to get them closer to 100- 150, is that a goal I can get to soon?

Wed Jun 20 2001 21:13:36 Eastern US
BettyB (Support Chat Moderator) from 198.81.16.49
I am not a fan of Mix25, and especially not for type 1. You give up too much needed flexibility in my opinion.

Nina (Diabetes Educator) from 148.221.179.58
Have you used Humalog for Spenser?? ... Yes Betty, i agree, thanks ... Hi Richard!!

Wed Jun 20 2001 21:15:21 Eastern US
Heidyn (pumper 2 months) from 164.106.22.64
Beth,I'd say the biggest thing for me was don't get stressed out if things don't automaticlly fall into place. It took me a month before I started seeing numbers in the right range, but you can get there!

Wed Jun 20 2001 21:15:23 Eastern US
Spensers mom from 24.218.5.194
We have changed Dr's. The first one was give insulin and eat, the other is give the insulin to as much food as he eats, So the insulin amount has gone down. He grows about 3 inches a month, so the Dr.'s are not worried about his weight, I am though.

Wed Jun 20 2001 21:15:56 Eastern US
Daniel (Pumper) from 64.20.226.143
I hope you took that as a compiment spensers mom, I certainly didnt not mean that he was too big.

Wed Jun 20 2001 21:16:13 Eastern US
Spensers mom from 24.218.5.194
Sorry that was 3 inches every 3 months, we check at the Dr.'s

Wed Jun 20 2001 21:16:44 Eastern US
BettyB (Support Chat Moderator) from 198.81.16.49
Spenser's mom, it is very hard to get stable blood sugar control in type 1 diabetes when you're taking the same insulin dose every day -- it's especially tough to get kids to be consistent in food intake, appetite, and physical activity day after day.

Wed Jun 20 2001 21:16:56 Eastern US
Beth (IL) (Ian, 3 1/2, dx'd at 2 1/2- starts pumping next month) from 65.6.41.52
Heidyn, that's what our doctor has said - to fully expect crummy numbers in the beginning.

Wed Jun 20 2001 21:16:58 Eastern US
BettyB (Support Chat Moderator) from 198.81.16.49
Your goal of 100-150 most of the time is definitly a reasonable one - but will probably require you to learn to adjust insulin for food and blood sugar.

Wed Jun 20 2001 21:17:04 Eastern US
Spensers mom from 24.218.5.194
No the Dr. never told us about Humalog, How does it work?

Wed Jun 20 2001 21:19:41 Eastern US
BettyB (Support Chat Moderator) from 198.81.16.49
With that rate of growth, Spenser's definitely getting enough insulin. He wouldn't be growing if things were too far out of whack!

Wed Jun 20 2001 21:20:51 Eastern US
Spensers mom from 24.218.5.194
Would you use Humalog and not R? Would I give the shot before each meal? We do not have a full time nurse at school. Only there 2 days a week, Great NH.

Wed Jun 20 2001 21:20:58 Eastern US
BettyB (Support Chat Moderator) from 198.81.16.49
Humalog is a rapid acting insulin - perfect for meals - acts fast and goes away quickly. It can improve after meal blood sugars, reduce lows between meals and overnight, and can be given at the END of the meal, so you can more carefully match insulin to actual food intake. Very cool! It's Lilly's version. Novo will be bringing it's version, called, Novolog, to the US market later this year.

The only thing the Dr. has said about insulin and that you gain weight, and I don't want to have him get too big. I worry too much I guess.

Wed Jun 20 2001 21:24:21 Eastern US
CrazyCajun from 66.44.42.39
If you eat normally and are on insulin, you will gain some weight, but not so much that it will make you fat

Wed Jun 20 2001 21:24:58 Eastern US
BettyB (Support Chat Moderator) from 198.81.16.49
depending on your comfort, Spenser's Mom, you might want to stick with N to cover lunch on school days, so Spenser doesn't have to take a shot at school. But you could switch to Humalog with each meal on the weekends and vacation when meal time and size are less predictable. If you were going to do that, you'd have a different morning N dose on school days.

Wed Jun 20 2001 21:25:44 Eastern US

Spenser has always been a big boy, born at 8,14, and 23 inches, Has always been off the charts, that is why the dr in not worried, He has only gained 8 lbs since dxd on 11-21-00.

I don't think Spenser is gaining abnormally. It seems to me Mom also said he was gaining a lot of height - in other words, he's GROWING - not just laying down fat stores.

Wed Jun 20 2001 21:29:02 Eastern US
CrazyCajun from 66.44.42.39
Spenser's mom - As we all know, there is a difference between big and fat. If he is naturally large, then he will most likely gain a few pounds on insulin, but if he eats a halfway decent diet and if he exercises, then he will still be normal(weightwise)

Betty, I FINALLY bought Sweet Kids! For the longest time, almost a year now, I would have to call the doc or RD to "check" to see if I was doing everything right. I hated to have to call for every little issue that I ran into. BUT, the endo's office really doesn't give any more info than what is necessary when we go to visit. It was like they has some secret about control and was not going to let me in on it. NOW that I have read Sweet Kids, I have some the "secret". Thanks so much for the work that you did!

Anne. None of this is rocket science -- and so much of the frustration comes from people not having enough information and tools and take an active role in solving problems day to day. For example, taking blood sugars for the doctor!! What a farce. They've lost most of their power to make a difference by the time you see the doctor weeks or months later.

Joey, I've not heard of that one specifically, but there ARE a number of skin and connective tissue disorders that are far more common in type 1 diabetes. Do you see and endo? That's something an endo who specializes in diabetes (not all of them do) might know.

Joey, most insulin allergies are manageable with desensitization treatments -- but a lot of docs will just try switching brands of insulin first. it's virtually never the insulin itself, but something like the rubber in the stopper or the preservative in that particular brand.

One problem along the lines of what we are discussing is mixing insulin when pumping. I have read, and some moms here mix R or Velosulin with H to help maintain a site for 3 days. I mentioned it to the endo, and he wants to see data on the practice before he will suggest it. Now, would I be wrong to try mixing even if he sugguested not to do it?

Wed Jun 20 2001 21:58:45 Eastern US
BettyB (Support Chat Moderator) from 198.81.16.49
Insulin allergy generally hows up first right around the injection site and then can become systemic and show up all over the body -- just on the arms doesn't sound like it unless that's where you're giving the shots.
