Doing good Betty, there's nothing like getting a good a1c to improve one's outlook on diabetes

Wed Sep 12 2001 21:04:10 Eastern US
BettyB (Chat Co-Host) from 64.12.102.22
Hi, Kathy. Kris and I are diabetes educators who have a center for patient and professional education in Phoenix, Arizona. I'm a dieitian and Kris is a nurse.

Kris and I do these "virtual support" chats - one for type 1 and one for type 2 - each month.

Wed Sep 12 2001 21:06:07 Eastern US
Kris (chat cohost) from 205.188.195.165
These are the 2 nights per month that my husband know I will cook dinner since Betty is with us!

Wed Sep 12 2001 21:06:18 Eastern US
BettyB (Chat Co-Host) from 64.12.102.22
The idea is to have a forum where you can get the usual support from other folks on the chat but also have a health professional to bounce questions off of if you like.

Wed Sep 12 2001 21:07:02 Eastern US
Kathy (Chris(M),16,pump) from 24.112.158.254
Chris had his first severe low reaction last week. Had 4 wisdom teeth out in the office and then went low about 2 hrs later. He was 11.2 then dropped to 0.8. Had to call 911. My question is does anthestic make the blood sugars drop.

Wed Sep 12 2001 21:07:08 Eastern US
Ellen from 64.12.106.37
What's cooking at Kris' house?

Wed Sep 12 2001 21:08:28 Eastern US
Kris (chat cohost) from 205.188.195.165
Ellen, the menu is pot roast, potatoes with fresh chives and salad with this wonderful vinegrette dressing we got in Orangeville, Ontario last weekend. yum yum...

Wed Sep 12 2001 21:09:35 Eastern US
Kathy (Chris(M),16,pump) from 24.112.158.254
Hey I live 45 minutes from Orangeville. I live in Brampton.

Wed Sep 12 2001 21:10:22 Eastern US
BettyB (Chat Co-Host) from 64.12.102.22
Kathy, to my knowledge the anesthetic is not the most likely candidate for the problems. Most likely is stress - some folks respond to stress with falling rather than rising blood sugars. Or, he RELAXED under the anesthesia, lowering insulin need. Also quite likely is the simple change in insulin requirements -- no food and a basal rate that was too much for such a long period without food. Does either of these sound likely to you?

Wed Sep 12 2001 21:11:05 Eastern US
Kris (chat cohost) from 205.188.195.165
Well, Kathy, we were just in your neighborhood last weekend, Toronto to Niagara on the Lake. We've been in Orangeville this summer too. The Oasis cafe is our favorite. We drove by the sign of Brampton and actually, we're drinking wine right now from that area.... we are bad girls, eh?

Wed Sep 12 2001 21:12:07 Eastern US
ms from 205.188.192.32
hi everyone - kathy, was he put under fully? I specifically did not want to be put under at all!!! I am a control freak sometimes - wanted to be in full control of my bgs. The office was really good though when i had mine out - they had told me to let them know when i wanted them to test me (in the event that i needed to be put under)

BettyB (Chat Co-Host) from 64.12.102.22
Hi, ms and FlowerPower - welcome to the chat!

Wed Sep 12 2001 21:13:03 Eastern US
Kathy (Chris(M),16,pump) from 24.112.158.254
all sounds likely to me. We did not change his basal rate as he had surgery before and just didnt bolus. I think the stress factor was probably our worst enemy here.

Wed Sep 12 2001 21:14:49 Eastern US
Kathy (Chris(M),16,pump) from 24.112.158.254
Well too bad I didnt know the two of you last weekend - I really could have done with some knowledgable help. Our hospital does not deal with pumps so we were winging it basically on our own. Yes Chris was put right out for his teeth.

Wed Sep 12 2001 21:16:25 Eastern US
*FlowerPower05* from 64.12.103.32
thanx a/s/l all

Wed Sep 12 2001 21:17:26 Eastern US
BettyB (Chat Co-Host) from 64.12.102.22
Many people with type 1 get to the point where they are adamant about not letting people who don't specialize in type 1 diabetes take over control of pump, insulin, etc.. and that includes virtually all usual hospital staff.

last spring she had a seizure during the night :-(

Wed Sep 12 2001 21:21:54 Eastern US
BettyB (Chat Co-Host) from 64.12.102.22
ms, our feeling is that each person is their OWN diabetes expert. There must be trust and knowledge that any person who would touch your pump knows YOU and what they are doing. That is seldom the case when working with unfamiliar health professionals in an inpatient situation.

Charla (Mom of Eric, 10, dx'd at 5, pumper) from 64.132.78.41
We complained about her several times. We were proactive and expressed our concerns before Eric began middle school in August. Met with the principal on Friday morning to discuss her lack of knowledge. She told Eric that I must have written it down wrong so she changed it herself even after Eric said she wasn't supposed to and she needed to call me. He did go low but not terribly. Eric didn't feel comfortable challenging her authority.

Wed Sep 12 2001 21:35:35 Eastern US
BettyB (Chat Co-Host) from 64.12.102.22
Mary, maybe the A1c went up becausd you got rid of lows. Could that be it. Looks discouraging, but if that's what happened it's actually the first step to much better life and control.

caroljean (mom of bryan 15) from 169.207.126.21

Wed Sep 12 2001 21:36:59 Eastern US
BettyB (Chat Co-Host) from 64.12.102.22
Charla, at 10 Eric is at a real disadvantage. Have you talked with the principal about the latest episode. and is there a WRITTEN plan for when she is to call you? That might help to eliminate her playing cowboy.

Wed Sep 12 2001 21:37:13 Eastern US
Mary (NE) (Kira's mom) from 65.28.165.116
Could be, BettyB. The more we tested, the more we realized how many lows we had probably missed.

Charla (Mom of Eric, 10, dx'd at 5, pumper) from 64.132.78.41
Eric went down to 62 and, considering his insulin ratio of 70:1, he would have been about 112 if she hadn't changed the number. She did call and admit what she did and said "I guessed I should have called you when I questioned it". DUH!

Ellen (FL) (Zachary 13) from 64.12.106.37
I think that nurse should be reported to the state board of medical whatever - she's practicing as a doctor without a license...she has no authority to make adjustments to your child's insulin dose.

Wed Sep 12 2001 21:38:38 Eastern US
BettyB (Chat Co-Host) from 64.12.102.22
That's definitely it, then, Mary. The A1c was being bought at the price of all those lows. Now you can start workin on the highs and bring it back where you want it. I want to congratulate you on that. Lows are such a burden for the family AND the child!

Wed Sep 12 2001 21:38:43 Eastern US
Charla (Mom of Eric, 10, dx'd at 5, pumper) from 64.132.78.41
Yes, Eric has a 504 Plan. I called teh school this afternoon and haven't heard back from the principal but Eric said she wasn't at school today. He had a sub nurse.

Charla (Mom of Eric, 10, dx'd at 5, pumper) from 64.132.78.41
I agree Ellen. 504 says only doctor and I are allowed to change insulin dosages. She should have known better because the item in question was sliced ham. She added 10gr since she thought I had written it down wrong.

Wed Sep 12 2001 21:41:19 Eastern US
BettyB (Chat Co-Host) from 64.12.102.22
That's understandable, Charla. Anger is the natural response - she messed asround with your child's safety! But, dependiong, she may be wiser for it now. Do you think she's the kind that learns from her mistakes?

She's been a school nurse for over 20 years and last year she had 4 kids with diabetes. This year she has 3. She is supposed to go for "basic training" in diabetes as per our demand on Friday.

Wed Sep 12 2001 21:43:49 Eastern US
BettyB (Chat Co-Host) from 64.12.102.22
Charla, it's surp[rising how LITTLE health some health professionals know about the nitty gritty of diabetes management. Sad to say, but true. It's never wrong to give them information at the lowest possible level of understanding. People who know more can understand.

Wed Sep 12 2001 21:44:51 Eastern US
BettyB (Chat Co-Host) from 64.12.102.22

I knew we were in trouble when she didn't know how to use Glucagon when I dropped it off the day before school started. All three kids she has this year go to the same doctor so she has to go to his office for training and training for pumpers. (Third session of pump training since Decemeber.) She thinks all the kids are the same and tries to equate pumping with MDI.

i have a question. i have been taking insulin for three weeks. for the first two weeks, my numbers were good. but i was also taking glucophage, because i was diagnosed incorrectly 3 mos. ago with type 2. a week ago, my doctor told me to stop taking glucophage, and now my numbers are significantly higher. i feel like i must need more insulin. should i call my doctor? i need some opinions.

Sylle, definitely talk with your doctor. Anytime your numbers are consistenly out of range, it makes sense to to change soemthing. And hopefully, with a little time you will eventually learn how to make such changes for yourself.

Wed Sep 12 2001 22:00:36 Eastern US
BettyB (Chat Co-Host) from 64.12.102.22
Caroljean, our experience so far with Lantus is that there are a few folks for whom it works really well - they seem to be people who neither have a strong Dawn Effect (which raises basal needs temporarily) nor do much exercise (which lowers basal insulin needs temporarily)People who need a more variable basal do better in our experience with NPH twice a day or with a pump.
