Question:
Our 14 year old daughter has had IDDM since age 5. This year she is concerned
about hood
she needs to cover her insulin. She has lowered her doses on exercise days
but still becomes hypoglycemic. She is frustrated. Any ideas?
Answer:
I would suggest you and your daughter discuss with your physician and
dietitian how to best balance her exercise and food to avoid low blood
sugars, and excessive weight gain (or weight loss). A few suggestions
to consider and discuss with your daughter and her Diabetes Team:

Sometimes you need to change an insulin other than the one you think
is working during the time of exercise. In other words, an insulin that
is supposed to be peaking before the exercise may carry over into the
time of exercise.
- Sometimes blood sugars go low 6 to 12 hours after exercise, and
insulin working at this time may need to be changed.
- There is no one ideal snack to eat as supplemental food before or
during exercise, but if your child has problems with weight gain. Frequent
small amounts of pure carbohydrate such as fruit may prevent lows as well
as one snack containing complex carbohydrate with protein and fat before
the exercise. With less fat and protein, you will have fewer calories.
However, it may not be practical to eat so frequently. One large pure
carbohydrate snack may raise the blood sugar too
quickly and not last long enough. Therefore, if you skip the protein
and fat, you will probably need several small snacks.
