Question:

From Bucharest, Romania:

My son is 9 years old, diagnosed with Type 1 diabetes 3 years ago. In case a hypoglycemic episode occurs just before one of his main meals (and insulin shots), when should the insulin be administered? Before or after eating?

On our next holiday, we are planning a trip to a warm country. Should we expect important changes in the values? Are there any specific changes needed in his diet or insulin treatment?

Answer:

The Regular (soluble) insulin is advised to be administered from 15 to 30 minutes before starting eating if the values of blood sugar are within normal values; in presence of low values, it is given just before eating. It is always better to give some soluble sugar such as juice or Coke before normal foods to rapid counteract low blood sugar.

Travelling to a warm country generally does not change insulin dosages or diet, unless, as it is common in warm climates, your son decreases his caloric intake and appropriate reductions in insulin dosages must be taken based on his blood sugar monitoring.

When using lispro insulin [Humalog® brand], it is perfectly okay to take the insulin dose after the meal, especially if the blood sugar level is low right before mealtime.
