In type 2 diabetes, do you usually develop ketosis prior to initial diagnosis? Is there any danger of someone with type 2 diabetes going into DKA?

Answer:

Prior to diagnosis of type 2 diabetes, when the blood sugars are elevated, you can see ketones in the urine. The ketones are produced because of the inability to use glucose as an energy source. This is not the same as DKA [diabetic ketoacidosis]. In DKA, you have increased ketones that result in systemic acidosis.

Only in rare situations do people with type 2 diabetes develop DKA. This is usually in the setting of a serious precipitating illness, such as severe infection or heart attack.
