From Sandy, Utah, USA:

For someone who currently takes Lantus at 2 p.m., but is traveling to Australia, which is 20 hours ahead of Utah, how should she alter the Lantus dose to account for the time difference? She takes Humalog before meals.

Answer:

For the first day, give the Lantus injection 20 to 24 hours after last injection and then transition to 2 p.m. Australia time when you have gotten on track to local time. There is no exact science to this. Humalog is given which each meal, regardless of time.
