Children with Diabetes is running a survey about issues surrounding transitioning from pediatric to adult care to help the National Diabetes Education Program, a federally-sponsored initiative of the NIH and CDC. We want to help prepare families navigate through the critical phase of transitioning from pediatric to adult diabetes health care, addressing topics such as managing your own health care needs, securing health insurance, finding a new adult health care provider, transferring medical records, and seeking appropriate health care as needed. This survey will help identify the critical issues, challenges (and some solutions), as well as the types of venues and materials to reach the target audience-parents of and teens with diabetes.
