You and your child are invited to participate in a study designed by researchers at the Child Study Center of Virginia Tech in Blacksburg, Virginia and the University of Kansas Medical Center. We are interested in better understanding the health, environmental, and social factors that may influence diabetes symptoms of children ages 11 to 15. To accomplish our goal, you and your child would be asked to complete several questionnaires that address your child's management of diabetes and your role in helping to manage your child's diabetes.
If you are interested in participating, please fill out the form below. I will send you the questionnaires for you and your child to fill out. Filling out the questionnaires will take about 30 minutes of your time. After I receive your completed questionnaires, I'll follow up with a phone interview that will be scheduled at a convenient time for you.
