In addressing the psycho-social issues of our children with diabetes, please take a moment and give a brief statement of single issue you feel is most important, and why. Give the age group of your child. If you do not see a category you feel is most important, please add it and briefly explain why it would be important.

5. Parent and Family issues: (the burden on the family, childcare, support for family such as counseling)

6. Healthcare issues: (gaps in current care and how those might be improved)
